Parkinson's Disease

Monday, February 8th

The MDS-UPDRS Part III remains the standard motor endpoint in early Parkinson's disease trials, yet a machine-learned measure from wrist accelerometry has been shown to detect disease progression in prodromal Parkinson’s Disease (PD) more sensitively than this scale — with wearable-sensors elsewhere reaching statistically significant, progression-consistent change within roughly ten months and larger two-year effect sizes than conventional clinical rating in early-stage disease.

This session challenges sponsors to reconsider whether episodic clinical scales alone are sensitive enough for early-population trials, then tailors the evidence to the room: biostatisticians will see the power and sample-size case for continuous digital endpoints; operations leads will see the wear-compliance and data-pipeline requirements; regulatory leads will see how composite construction (reference-based vs. reference-free) affects qualification strength.

The session closes with a clear position: in early and phase I/II PD trials, sensor-based composite measures are a validated complement to clinical rating, not an exploratory add-on — and omitting one risks underpowering detection of true treatment effects.
